The other version, and the box that decides it

There is a light fixture there, or nothing. Now the job includes a box that can actually hold a fan.

NEC 314.27(C) is the section. An outlet box used as the sole support of a ceiling-suspended fan has to be listed for that use and marked for it. A standard lighting box is not, and the reason is mechanical rather than bureaucratic: a light fixture hangs still and a fan vibrates continuously for years. Fan-rated boxes carry a weight rating, commonly up to 70 pounds for a ceiling paddle fan, and a heavy fan on a long downrod is exactly the case where that number needs reading rather than assuming.

Getting a rated box into an existing ceiling is where the labor is. If there is attic access above, it is reasonable. If there is a second storey above it, somebody is opening the ceiling and closing it again, and that is a real part of the price.

  • Wall-mounted fan speed control. Needs a control listed for a fan motor rather than an ordinary dimmer, which is a common and damaging substitution.

That last one is worth stating plainly. A standard incandescent dimmer is not a fan speed control. Putting one on a fan motor makes it hum and shortens its life.

What actually moves the number

  • Whether a fan-rated box is already there.
  • Ceiling height, because anything above a standard reach needs different access.
  • Whether there is attic access above the location or a finished floor.
  • Sloped or vaulted ceilings, which need a compatible mount and a longer downrod.
  • Whether you want the fan and the light on separate switches.
  • How many, since the second and third fan in one visit cost less each than the first.

If the ceiling is going to be open anyway, that is the moment to think about anything else that wants a box in it, because the expensive part of the work is access rather than the device.
