Walk through a house in Monte Vista, Terrell Hills, or any of the older pockets on the near north side and you will find outlets with two slots instead of three. Homeowners tend to treat this as either nothing at all or as proof the house needs to be gutted. It is neither. It is a specific, well-understood condition with three legal answers, and picking the right one is mostly a question of what you plug in and what else the wiring is doing.

What a two prong outlet is telling you

The third slot on a modern receptacle connects to the equipment grounding conductor, a wire that runs back to the panel and does nothing at all under normal operation. Its entire purpose is to give fault current a low resistance path home if a hot conductor ever touches the metal case of an appliance. With that path present, the fault current is large, the breaker opens, and the appliance case never becomes dangerous to touch.

Wiring installed before that conductor was required simply does not have it. There is no third slot because there is nothing to connect it to. The outlet is not broken and the circuit is not failing. It was built to a standard that has since moved.

Why it matters more now than it did then

Because of what you plug into it. A lamp and a radio were fine on an ungrounded circuit, which is why the standard held for decades. A metal-cased power tool in the garage, a window unit, a computer, or anything with a three prong plug is a different situation, and the adapters people use to force a three prong plug into a two slot outlet defeat the protection entirely unless the adapter tab is bonded to a grounded box, which in an ungrounded system it is not.

There is a second issue that gets less attention. Surge protective devices at the outlet, the power strips people put behind a television, work by diverting excess voltage to ground. On an ungrounded circuit there is nowhere for it to go. The strip's indicator light may still glow, so the homeowner believes equipment is protected when it is not.

The three legal fixes

The National Electrical Code addresses this directly in 406.4(D)(2), and it gives more room than most people expect.

The first option is to leave a two prong receptacle in place and replace it with another two prong receptacle. This is permitted. It is honest, it costs almost nothing, and it keeps the circuit clearly marked as what it is. It is a reasonable answer for a bedroom outlet that runs a lamp and has for sixty years.

The second option is a GFCI receptacle, marked No Equipment Ground. This is the option that solves the shock hazard, and it is the workhorse fix in older homes. The GFCI watches for current leaving the circuit through any path it should not take, including through a person, and opens in a fraction of a second. It does not need the equipment grounding conductor to do that, which is exactly why the code allows it here. The marking is not optional, and it is not decoration: it tells the next person that this outlet has shock protection but no ground path.

The third option is to run an equipment grounding conductor and make the circuit a genuinely grounded one. This is the complete answer, it is the one that restores surge protection and satisfies equipment that needs a real ground, and it is the most work. Sometimes it is a short pull. Sometimes it means opening finishes.

The one option that is not on the list

Replacing a two prong outlet with an ordinary three prong outlet and leaving the ground terminal empty. It is not permitted, and it is worse than doing nothing, because it is a lie told to every device plugged into it. The plug fits, the appliance assumes a fault path exists, and the person holding the drill finds out otherwise.

If you bought an older San Antonio home and the outlets are three prong, do not assume the house was rewired. A three dollar plug-in tester will tell you whether the ground is real. Reading open ground on an outlet that looks modern is common, and it is worth knowing before you plug in something that matters.

How this connects to the rest of the house

Ungrounded outlets rarely travel alone. A home wired before grounding was required usually also has an original service, a panel that is full, cloth-insulated conductors in the attic, and no GFCI protection anywhere near water. None of that is an emergency by itself. All of it is worth knowing about together, because the sensible order of work changes depending on what else is going on.

That is the case where an inspection earns its cost. If the wiring insulation is sound and the service is adequate, targeted GFCI protection is a cheap, correct answer and you are done. If the conductors are brittle where they enter boxes, the outlets are the least of it and the money belongs elsewhere.
